Regenerative Therapy


WHAT IS BMAC?

Bone Marrow Aspirate Concentrate (BMAC) is a regenerative therapy procedure that uses cells from a patient’s bone marrow to initiate healing for a number of orthopedic conditions, including osteoarthritis and tendon/ligament injuries. It also contains a mixture of white blood cells (including progenitor cells, lymphocytes, and granulocytes) and platelets, as well as growth factors and proteins. Bone marrow aspirate concentrate (BMAC) treatment uses a patient’s own bone marrow aspirate for its potential to reduce inflammation and help restore normal function in joints and tendons/ligaments in patients. 

Chronically painful areas often have inadequate blood supply, healing potential, and regenerative cells to repair damage. These regenerative cells taken from the bone marrow can actually replicate themselves into various types of tissue. This in turn gives the patient a robust super concentrated mixture of regenerative cells that have very accelerated healing abilities that repair and aid in growth in the damaged area.

It has tremendous potential for patients with orthopedic conditions or injuries and is most often used to treat moderate to severe arthritis in the shoulder, knee, ankle and hip.

It has also been successful in the treatment of tennis/golfer’s elbow, meniscus tears, low back pain from disc herniation, rotator cuff tendiniti, achilles tendinitis

BMAC is obtained during an outpatient procedure in which a small amount of the patient’s bone marrow is extracted from the pelvis, using local anesthesia to minimize any discomfort. The marrow is then processed in a centrifuge to produce a concentration of regenerative cells and growth factors which will then be injected into the patient’s damaged tissue under ultrasound guidance to promote healing.

The procedure involves minimal downtime and patients typically feel sore for two or three days at the site of the injection. As with all regenerative procedures, anti-inflammatory medications such as ibuprofen, naproxen, and aspirin should be avoided as they may interrupt the healing process. Physical therapy can be started in 2 weeks after the procedure. It may take 6-12 weeks to see the benefits of BMAC


Adipose fat therapy


Adipose/Fat therapy is a gentle process that uses your body’s own fat tissue to cushion and support areas of injury or damage as your body heals itself. The adipose or fat is usually obtained from the stomach/adomen areas using a local anesthetic. Adipose/Fat therapies rinse, cleanse, and re-sizes the patient’s harvested fat. This harvested fat (referred to as MFAT which means microfragmented adipose tissue) is then precisely injected into the patient’s injured areas with the use of ultrasound guidance. Fat has regenerative cells, growth factors, and healing proteins known as cytokines. This helps with repair and healing of the tissue. 


 It is especially effective for areas where there isarthritis or tissue injury such as shoulder partial rotator cuff tears, knee patella multi area tendon tears, labral tears, meniscal tears, and arthritis of the knee, shoulder and hip. 


Procedure and Healing:

Adipose Fat therapies only require a small amount of fat to be removed. First a mixture of saline and an anesthetic is injected into the harvest area by the abdomen or stomach which will thoroughly numb it as well as loosen the fat tissue for removal. The solution also helps to minimize bruising/bleeding in that area. The adipose tissue is then suctioned from the patient’s using a probe. 

Many times, platelet rich plasma is added to the adipose fat therapy to promote blood flow and added repair to the area.

a2m treatment


Alpha-2-Macroglobulin (A2M) is a naturally-occurring molecule found in your blood. It acts as a powerful protector against cartilage breakdown by inhibiting the actions of proteins that damage joints and can cause inflammation.

A2M blocks proteins ( Matrix Metalloproteinases and A Disintegrin and Metalloproteinase with Thrombospondin) that are responsible for creating arthritis in the body. By harvesting a patient’s A2M and re-injecting the super concentrated A2M mixture into the diseased joint, the progression of arthritis (specifically osteoarthritis) can be stopped as well as preventing further loss of cartilage, promoting tissue growth and relief from inflammation/pain.

Extraction

During an in-office visit, a blood sample is drawn and run through a 

centrifuge to separate its components.

Concentration

The Solution is filtered through a proprietary system to isolate the A2M 

molecules for a concentrated dose.

Activation

The optimal solution is an injection under ultrasound guidance into your injury site to begin baiting 

and trapping the destructive proteins known to cause inflammation.

Healing Process

After the procedure, the patient will experience minimal discomfort for 3-5 days. Patients may begin to experience pain relief in 4-6 weeks. Patients are monitored by the doctor until the patient can completely return to pre-procedural activities. The regenerative process continues to work over a period of 6 months to a year with most patients continuing to improve in function/movement and decreasing in inflammation/pain.
